Agile EP includes several questionnaire options so you can match the intake process to your workflow and the client’s needs.

Each questionnaire serves a slightly different purpose. The sections below explain when to use each one.
Do I need to create different questionnaires for different client types?
You do not need to select a different questionnaire based on whether a client is single, married, partnered, or otherwise. Agile EP questionnaires are dynamic, so they'll adjust automatically based on the client’s answers.
For example:
If a client indicates they are married, the questionnaire will ask for spouse information and related details.
If a client indicates they are unmarried but partnered, it will adjust to ask about a partner.
If a client is single, it will skip spouse-related sections entirely.
If a client has children, additional child-related questions appear.
If they do not, those sections remain hidden.
The questionnaire logic adapts in real time to the client’s responses, which means you do not need separate versions for:
Married vs. unmarried clients
Clients with or without children
Different household structures
The same questionnaire intelligently adjusts to fit the client’s situation.
Questionnaire types
Client Questionnaire – EP with Fiduciaries
Best for: A comprehensive intake and client thought exercise.
This is the most complete questionnaire. It includes:
Personal and family information
Assets and liabilities
Fiduciary appointments (Personal Representative, Trustee, Agents, etc.)
Estate planning goals
Importantly, the selections made by the client for their fiduciaries are not saved in the Answer File for use in drafting. They are only included in the questionniare output form. This section is designed primarily as a structured intake and thought exercise for the client.
Clients work through key decisions in advance, which helps them:
Reflect on fiduciary choices
Consider distribution planning
Organize financial information
Arrive at meetings better prepared
This version is ideal if you want clients to think through their planning decisions before drafting begins.
Client Questionnaire – EP No Fiduciaries
Best for: Core intake when fiduciary decisions will be handled separately or if you prefer to discuss these roles before asking a client to name individuals who can serve in those roles.
This version gathers personal and financial information but does not collect fiduciary appointments.
You might use this if:
You prefer to discuss fiduciary roles during a meeting.
You handle fiduciary selections directly in the drafting interview.
You want to simplify the client’s initial intake process.
Client Questionnaire – Assets and Liabilities Only
Best for: Limited financial data collection or updating assets for existing clients.
This shorter questionnaire focuses exclusively on assets and liabilities. You might use this when:
Updating an existing plan
Conducting a review
Gathering financial information for funding analysis
It does not collect full estate planning or fiduciary information.
Client Questionnaire – Blank (To Be Printed or Emailed, Not Shared)
Best for: Paper-based or offline intake.
This version is designed to be printed or emailed as a static document rather than shared as an online interview link. Use this if:
A client prefers a printable format
You want to email a fillable document
You are collecting information outside the online questionnaire system
It is not intended to be shared using the standard questionnaire link feature and will not capture any data to be used for drafting at all.
