New: Build Your Own Questionnaire

The Build Your Own Questionnaire lets you choose which sections a client completes, so each client answers only what you need for their plan. It is available in Washington and Oregon. Build Your Own includes every question in the other questionnaires and will replace them over the next few months.

How it works

  1. Open the Build Your Own EP Questionnaire. The first page, Questionnaire Setup, is for you. Your client never sees it.
  2. Under Choose What Your Client Answers, check the sections you want. Personal information is always included, and Select / Unselect All starts you from everything or nothing. The sections are grouped:
    • People: children, other family members, additional people and organizations, and advisors. The first three also fill the list of people you choose from when drafting.
    • The plan: current estate plan, goals, and fiduciary choices. If you include fiduciary choices, pick which roles to ask about: personal representatives and trustees, financial agents, health care agents, the agent for disposition of remains, and guardians for minor children.
    • Health care and final wishes: in Washington, health care, end of life and final arrangements are three separate choices. In Oregon, health care preferences and final arrangements.
    • Finances: assets and liabilities. The financial summary page is included when both are checked.
  3. Under Choose Who Receives the Answers, pick the attorney responsible for the client and choose whether to email a copy of the completed questionnaire to someone at your firm.
  4. Click Share Interview at the top right. Enter your client’s email address, choose a password, and choose how long the link stays open.

Your client sees only the sections you checked, and the completed questionnaire prints only those sections.

Previewing questions and reviewing answers. When you open the questionnaire you see only the setup page. To preview the questions, or to review answers a client sent back, choose Yes under See What Your Client Sees. To review or change a client’s answers, load them first.
